I am on long term disability for 15 years.
Every year during all these 15 years, i did receive Two forms to be filed:
1. Client's supplementary statement,
2. Attending physician's supplementary statement.
However this year I have received just one single form, namely
only Client's supplementary statement form without other one
suppose to be filed by my doctor.
My doctor said, he did not receive any form form Insurance.
With your experience and practice, how that can be explained ?

Thanks for your question. There could be several reasons for this, including an error in their records or timing changes. If you're unsure, you can certainly ask your insurer if they require this form again.
