Add Comment Question: I am on long term disability for 15 years. Every year during all these 15 years, i did receive Two forms to be filed: 1. Client's supplementary statement, 2. Attending physician's supplementary statement. However this year I have received just one single form, namely only Client's supplementary statement form without other one suppose to be filed by my doctor.
