Hi. I am submitting forms to move from STD to LTD. My doctor and psychologist have determined I am not able to return to work due to an adjustment reaction as well as anxiety, insomnia, depression and concentration difficulties. The form only ask "What aspects of your job are you able to do?" and "What is/was the medical condition causing your absence from work?". What should I respond here? My doctor noted the above and my psychologist has more robust answers.

The form also asks "Have you applied for, or are you receiving any income either as a result of your disability or otherwise? " and notes STD as one option, do I note yes as I have been on STD?

Thank you.

Typically, you'll want to explain your condition and how it renders you unable to do aspects of your job. Ie. can't do A job duty because of B condition.
For the medical condition, you'll want to just explain what your diagnosis' are.
It's important to answer all questions to the best of your ability and honestly. If you're receiving income and they ask about it, it should be noted to avoid any issues down the road.
If you run into any difficulties at all, or have any other questions, please give us a call and we can discuss (at no charge)
