Did it result in an absence from work? Yes or No CPPD 2009-2012
If yes please discribe, include dates (yyyy/mm/dd) and treatment provided

Dr. Note states serious medical condition, which prevents him from partaking in any work at this time. He will continue to be absent for an indefinite period of time

Of course I will be honest, just need help with the wording

My Doctor is approving of my applying for LTD and could/would write a supportive letter regarding the past and on going depression

My concern is Pre Existing Condition

I returned to work to be a productive member of society.

Now the depression has worsen and I need to be off work to heal
