I am off on LTD. My dr note to work states that i wil not be able to return to my place of employment due to my condition. Will this statement enable my work to terminate and replace me and if so am i entitled to any severance? Also am i required to provide return to work medical questionaire wich the company wants my dr to complete. I have already provide a dr letter wich states i will ne off until at least Dec. 31/23

If your doctor does not believe that you will be able to return to work in the foreseeable future, your employment may be considered frustrated and you would be entitled to receive your minimum payments under the ESA. However, if your doctor feels that you may still be able to return to work with further treatment, then your employment would not be considered frustrated.
