Late April/May 2022 was an approved STD leave due to a traumatic incident (psychosis)
Returned back to the role around July with a 8 week transitionary plan, and 100% to resume up to expectations.
October 2022: Requested second STD leave and approved as work-related incidents was deteriorating my health even further and risking into psychosis (1st STD leave)
November 2022, returning back to work, I was assumed to work at 100% capacity based on company’s insurance policy’s conclusion. I was also put on a performance management plan, which I was unable to meet due to my mental health challenges from the aftermath of April’s incident .This was disclosed that if plan requirements were not met, the company can pursue further measures.
January 2023- disclosed to the company on an incident unrelated to work (accidental pregnancy and miscarriage), that further deteriorated my well-being. Given my 2022 annual performance review was marked as poor, and my options of taking some time from this was going on unpaid absences. I have also had a few other absences after because I was struggling to reach out where to seek for guidance along with keeping up my mental health and my employment
December 2023: inquired on accommodation options and case was closed as the case was closed given that I was advised on February 2023 the accommodation options are very limited and my asks were rejected. This meant I was to be expected to perform at 100%.
January 2023: received a formal warning letter
March 2023: requested a 3rd STD leave. Note- my mental health was further slowly deteriorated to the point I wanted to quit. Even a STD was not enough time to engage in a plan to get me back to 100% working capacity in my role due to the timeline. I received a no contact formal notice from the company on March 30, I was transitioned over LTD leave, starting May 7th 2023. My LTD states the end date is January 2025.
Recently they called in to ask for medical updates and have provided me with a tight timeline of September 16 of inquiring for medical documents from being on LTD leave till now.
