I am on Ei sickness benefit
The employer is asking for a functional abilities form to be filled out and be sent back by a certain date, and in the letter from the employer they stated if I should not return that form that my employment may be terminated for cause
Can they do that ?
I personally take this as a treat ?

Typically, employers can ask for these sorts of functional abilities forms while someone is on sick leave. I understand their phrasing is alarming, however they seem to just be warning that failing to cooperate with a reasonable request could have employment consequences.
