I have DTC full 10 years in the rears back to 2013 also CPPD Starting Dec 1st of 2023 my COD July 1 my insurance Mailulife sent my a very very genic consent form to sign Will I be out of line to ask them to send me anther one to sign to be more specific to my case with a list of my care givers this adds way to much tress hoping you help me out
