I keep getting asked to get papers filled out from my doctor (which is expensive and out of pocket ). Do i need to keep getting them filled out? I have multiple sclerosis and my doctor keeps telling them that i will never go back to work

Im sorry to hear this is happening. You can ask your insurer if you can provide authorization for them to communicate directly with your doctor - they would then typically cover the fees for the updates.
