I have applied for LTD and had my doctor fill out the main application but I have no other supplemental documentation. Im wondering if submitting a log of my symptoms while ive been sick would be helpful.

Providing something you've written yourself, outlining your symptoms and limitations, can sometimes be helpful. You can also consider submitting your prescription summaries (if you're in fact on any medications), and clinical notes from treatment providers such as psychotherapists or physiotherapists (if any).
