Are ltd adjusters allowed to request information from my doctor directly, without going through me?

Your insurer may be able to do this if you've given them signed authorization to speak directly to your doctors. If you haven't done this, but want your insurer to go directly to your doctor, you can ask if they will provide you with such authorization form to sign.
