I have been on LTD for 4 years due to mental illness. Last summer was the first time Ive received the APS forms to be completed, after the 2 years mark. So far, I have not received the APS this year. the insurance approval letter indicates they will be requesting information from time to time. Should I reach out to the insurance and ask them when will they send the APS next time? is it common, they will not ask me to fill up the APS on a yearly basis? how about if the mail got lost?
