After 3.5 years on LTD I received a Claimants statement of continuing disability, and other form for my doctor. They ask to describe changes in my condition and any hospitalizations. Do I need to include hospitalizations not related to the claimed condition? They ask if any further investigations planned. At this time there are none, but we don't know in the future, they want black and white and I don't know the future. How to fill it? Also not sure how to fill the "state your daily routine" What does the insurance expect? how detail I need to be? Do I need to write hour by hour? Some days I can do more things than others, what day do I describe? any tips on filling this? They also ask if I am capable of driving. Driving makes my pain worse, but I have driven no more than 10 times in a year, and short distances only. I am scared of being on disability for the rest of my "productive" years (I am in my 40's), they ask if I have "retrain" or plan to retrain and give details. I haven't retrained, but I don't know if in the future I will be able to and I'd be open to do so if there is a job I can do with my condition and limitations when/if ever ready in the future. How do I respond to this? I am so confused. Thank you in advance for your help!
