I was on modified duties for a shoulder injury, one night at home my nose started to bleed so bad it required medical attention. I live in northern Ontario, I was air lifted to Toronto and had surgery to stop the bleeding. Before I could recuperate which would have been two weeks for the packing to stop dripping out of my nose, and the severe migranes to stop my employer issued me a Record Of Employment saying I quit, and in the comment box entered the remark no show/no contact. All of this is fabricated by the employer to terminate my employment there was lots of contact between myself and the employer.

Your employer should not have done this. It is very very important that you contact me as soon as possible and I will put you in touch with one of our employment lawyers to discuss your options. You are owed severance and potentially human rights damages. My number and email are both below.
