Manulife wants me to complete CPP Disability forms however the version of the application they sent me (2015) is different from the version on the Service Canada website (2019). Should I go ahead and complete the forms from Manulife or will completing an older version of the forms result in Service Canada not approving my disability application? (I did contact Manulife about the different forms and they told me to fill out the forms that they sent me (2015).)

I would use the forms provided by the insurer if those are their instructions.
