I've been on long-term disability for the past 8 years. Long term disability insurance company is fully aware that I am paralyzed and not able to return back to my job or any job. Yet they send me these letters to fill out in regards to my condition stating that they will cut my benefits if I don't fill out this medical questionnaire assessment I seem to get one every year it's quite annoying is it mandated or do I have to fill this out keep in mind I am paralyzed not even able to open a envelope I have to get my PSW to open my mail. Do I have to fill out this long term disability claim form again.?

You are very right to be frustrated by the insurer’s repeated requests. That said, if you don’t provide the updates they may cut you off so practically speaking you may want to continue providing these filled out forms.
