I am reaching out to seek your guidance regarding two documents I have received from my new long-term disability (LTD) insurer, which are required to be signed and returned promptly. 1. Authorization to Communicate by Email: This document states that the insurer cannot guarantee the integrity and security of information communicated via email. It also includes a disclaimer indicating that the insurer will not be responsible for any loss or damages I may incur as a result of using email for communications. While I understand the need for this authorization, I am concerned about the risks associated with transmitting sensitive information through email. 2. Agreement, Authorization and Acknowledgement: This document requires me to confirm that all verbal or written statements made regarding my claim are accurate to the best of my knowledge. It further states that my claim could be denied or terminated if I fail to comply. Additionally, by signing this document, I would be consenting to the investigation of my claim and authorizing the insurer and several other parties to collect, use, maintain, and disclose my personal information for various purposes related to benefit plan administration and claims management. My primary concern is that signing these documents seems to serve the interests of the insurer without providing any clear benefit to me. Additionally, I worry that the provisions could potentially be used against me in the future. Given that my well-being may not be their primary concern, I am hesitant to proceed without fully understanding the implications. I would appreciate your advice on whether I am obligated to sign these documents, what my options are if I choose not to sign, and any potential risks involved in either decision. Thank you for your assistance.
