I have seen several people on this forum that are on ltd for several years >2 years for mental issues but i know that insurance companies like to cut you off after the 2 year mark. if you are still unable to work do they still have to pay you and does the employer have to still keep you on and pay for medical benefits?

Yes if you are unable to work in any occupation for which you have training, education or experience after 2 years then they MUST pay you (unless you've reached the age limit on the policy, which is typically 65). And your employer must keep you on or pay you severance. We deal with issues of this nature each and every day at our office and help people get the compensation they are entitled to by law.
