My doctor is off on disability himself, I have seen a sub doctor that is covering, I had a disability form filled out and it turns out it was the wrong form, I asked to get the correct one completed they don’t seem to giving me a hard time to and in getting the correct short term form completed.i am on sick leave and my 30 elimination days at work are over in July 2nd, I haven’t submitted anything to my benefit company as yet to start the claim for short term disability. Can I change to new family doctor without it causing a red flag? My husband just started with this new doctor and he said he would not have an issue doing the disability forms.
