I am on DI appeal process while on EI sickness benefit (today was the last day) what is my obligation to the insurance company in regards to telephone interviews. Can I request a written questionnaire to be done by the insurance company? She left me a voicemail saying that they want to have a interview asap to process my appeal request.

If you are referring to an insurance disability claim then you have an obligation to answer their questions. If they insist on a telephone interview you can request to have the questions in writing but if they refuse then you risk them denying your appeal.
