My insurance company gave me a deadline to respond to them to accept to go to occupational therapy but also gave a caveat that if i cannot go i need a doctor's explanation to contradict why not. My physician wrote a letter to them indicating i am not ready to go back to work and sent it to them prob a day after I emailed them to let them know i contacted my doctors. Immediately after I emailed them just indicating i have contacted my doctors and NOT agreeing or disagreeing to seeing a occupational therapist they send me a letter saying my case is closed and i need to appeal. Can they do that and what is an appeal?

No they can’t do that. Call or email me ASAP at my contact info below and we will discuss your legal options. Don’t bother with an appeal. They rarely work in my experience and just prolong the amount of time the insurance company doesn’t pay you.
