I had an accident in May of 2015 and my fingers where cut off. On the pinky, ring finger and middle finger they where cut below the second clang and the index finger was cut at the first flange. I have disability/ dismemberment insurance at work however the insurance company is deny my claim. They said my policy states I must cut four fingers - two or more flanges. I have lost three fingers more than two flanges and one finger (index finger) just one flange. Can I apeak this decision?

I am not clear from your question what the policy states and what exactly happened to you. We will need to analyze the wording of the eligibility criteria of the policy and understand exactly what happened to you. We will then be able to tell you what your options are.
